PC Users: If you do not have a PDF reader, you can download the most current version of Adobe Reader for the PC. The PC PDF Registration Form is available for you to complete in your PDF reader, or to print and then complete by hand. You can submit your form via email as an attachment, or you can mail the form. Note that if you choose to email your registration, you will not be registered until payment is received.
Macintosh Users: If you do not have a PDF reader, you can download the most current version of Adobe Reader for the Mac. If you do not wish to install Adobe Reader, you may use the Safari PDF reader plug-in for the Mac. At this time, the fill-in feature of the PDF is unavailable, and we do apologize for this inconvenience. Please print the Macintosh PDF Registration Form and mail it to us via the postal service.
You will receive confirmation that your emailed registration form has been received within two business days. If you do not receive this confirmation, please contact us either by telephone at (317) 278-2385 or by email. If you choose to submit via the postal service, please allow us seven days to send confirmation.
